Health Services Manager
The Nurse Manager will assist CWI management staff by creating an environment to lead CWI’s professional nurse efforts in a planned, systematic, and on-going process and have basic responsibility for the health care of consumers to ensure the quality, adequacy and appropriateness of health services for the consumers we serve. Actively cooperate with the President/CEO and other staff members in a joint endeavor in satisfactorily achieving the goals of CWI.
Registered Nuse with New York State License, with a minimum of three years experience in OPWDD nursing, or other similar venue, and one year supervisory experience. Person with documented leadership and organizational skills preferred. Must also have a clean driver’s license and be eligible for 19A/CDL certification. Salaried position, contact Human Resources.
Full Time, Exempt, On Call Rotation w/ stipend, Location in the Southern Adirondacks
Salary commensurate with experience starting in the mid $30's
The Residential Manager reports to the Division Director of Residential Services or Program Coordinator Designee, Vice President/COO & President/CEO. The position concept dictates that the Residential Manager will be responsible for day-to-day operations of one or more Individualized Residential Alternatives (IRAs) including ensuring consumers receive quality care via the planning and delivery of individual services. A Residential Manager will have primary responsibility for at least one IRA, including Habilitation Plan development/ implementation and associated progress documentation, and additional support responsibilities to the residential division as a whole. A Bachelors Degree in Human Services or related field or RN, and two years experience working with the developmentally disabled population, one year of which must have been in a supervisory capacity. Must be capable of handling all position responsibilities outlined in the job description, including leadership role, scheduling, problem solving, direct care, and accountability for quality service provisions. Must also have acceptable driving record.
Home Care Assistant
The Home Care Assistant will assist the Home Care Coordinator in providing residential habilitation and respite services, typcial shifts include nights and weekends. Home Care Assistants must have a HS Diploma/GED, good character and background, and a clean drivers license. Demonstrated competency in 1st Aid/CPR, safety procedures, Medication Administration, a plus. Must be highly responsible, able to work independently, with at least one year of experience working with the disabled population, Per Diem, $12/hr
The position of Residential Assistant requires HS Diploma,GED or Valid CNA and NYS drivers license acceptable to Agency insurance standards. Must be of good character and background, and display an affinity for working with people with disabilities. Base Salary $13.36/hr, 2nd Shift & 3rd Shift Differentials applied or $10.45/hr (Per Diem Residential Relief Assistant)

Per Diem, Upt to 40 hrs/wk, Varies based on needs

Position based at Transit Connection in Hudson Falls. High School Diploma or equivalent. Must be of good character and background and display an affinity for working with individuals with disabilities. Must possess a Class B CDL driver’s license and have three years of accident and violation free driving.
